Credentialing

  • Credential new and active medical staff (licensing, board certificates, affiliations, etc.) using computer software programs such as PMIS, EPIC, EXCEL
  • Update all dates, addresses, medical licenses, insurance policies, etc. in our credentialing software known as PMIS.
  • Ability to handle many difficult situations and complete major projects with little to no supervision.
  • Make sure we are up to date with medical staff bylaws and procedures.
  • On a monthly basis, complete numerous detail orientated reappointment packets by following up with all application statuses via phone call or email, and make sure everything is up to par.
  • Delegate them to each provider via mail or email to be completed.
  • Regularly receive reappointment packets back for review and finalization.
  • Verify Privileges and competencies and compare them to their privilege log to make sure they have done the right amount in order to be granted the privileges they are requesting or upgrade a privilege otherwise the privilege can be downgraded if the provider does not show the correct proof, or I can not find privilege in the providers log.
  • Confirm providers reapplication such as their: NPDB report, AMA report, Board Certification, DEA, Medical Licenses, OIG report, risk management letters, OPPE information, and any else pertaining to their reapplication renewal.
  • Maintain and track FNP Supervising Physician information.
  • Attend credential meetings, write minutes using Microsoft Word and prepare an agenda for the month, and often work with committee chairs to better the credentialing process.
  • Send and respond to affiliation requests using fax machine, email and Microsoft Word.
  • Track all data on an MS Excel spreadsheet, so that all my information is to date, organized, and all info is correct.
  • Make sure to always be up to date with Medical Staff Bylaws and procedures.
  • Follow up via email and/or telephone calls on application statuses.
  • Send out OPPE forms for completion every 6 months and make sure peer reviews are up to satisfaction and are signed by each department chair, and maintain the OPPE'S for those months.
  • Filing paperwork and replenishing when needed, along with faxing, and collecting data such as subpoenas, time-sheets, mail, etc.
  • Meet with Credentials Board of Supervisors, joint commission and MEC once a month to talk about new applicants and reappointment's that are coming on for that month for final approval or denial.
  • Customarily assisting with answering The Medical Staff Office phone calls.
  • Create monthly staffing calendars.
  • Create monthly staffing schedules.
  • Run licensing and adverse action reports.

Provider Staffing

  • Established and maintained effective communication with staff, physicians and community organizations to promote high quality patient care.
  • Create calendars through Microsoft Word, cancel and create clinics. For the MD’S, DO’S, FNP’S, AND NP'S. For multiple different county’s such as: West County Health Center, Martinez Wellness Center, Martinez Health Center, Brentwood Health Center, Antioch Health Center, and North Richmond Health Center, with multiple deadlines, by using EPIC, PMIS and Outlook.
  • Often processing Very detail-oriented work Using excel spreadsheets and Microsoft Word.
  • Fill out A323’S for providers time off and send them via email to our appointment unit, the health center in which they were working at and to the provider themselves.
  • Always making sure there is a paper trail if needed and making sure the provider's time off or their created clinic request is done by the day it was requested.
  • Complete our C&R Calendars which were done through Microsoft Word, by either taking the providers off to show that they would not be in that day or adding them on if they were creating clinic’s, or if a provider was filling in for another provider.
  • Also producing a QA (Quality Assurance Check) I would go back through each week, making sure everything is correct and completed by not only us but also checking the appointment unit's side of the requests as well.
  • Furthermore, I would back-fill providers, making sure that there was always a provider on-call if needed for multiple health centers along with specialties.
  • Make sure all Newborn calendars for only the newborn doctors that needed to work certain Newborn shifts were processed.
  • In conjunction with the Newborn Calendars, I would work on the Extended Eve Calendars, where I needed to make sure there was always a doctor for the Evening shifts and help by creating calendars for multiple providers.
  • Made sure Ophthalmology and Urology always had an on-call doctor for each day for those Calendars as well.
  • Invariably emailing and calling providers, health centers, and appointment units, making sure they are tended to continuously.

On-Boarding

  • Among other things On Boarding can additionally be very detailed, it takes standardized steps to originate our new providers, locums, temporary providers and very seldom our replicants, with access through UAF forms which is implemented through databases such as VPN access, Email Access, E-prescribing for releasing medications, Imprivata, Pave, and EPIC.
  • Getting provider's lab coats and badges ordered using our email system Outlook and PAVE.
  • Applying online snipping tools to copy and paste Items that needed to be pasted from our systems such as EPIC and PAVE for example onto word documents or Power Points or calendars.
  • Working closely with the New applications to make sure we are comparable as far as hiring and making sure our providers get the complete treatment, they need to make them appeased, satisfied, and at ease when they come to work for us at CCRMC.
  • Making certain all transactions between the physicians and I, run smoothly with expectantly no complications.

Credentialing Specialist

  • Mail out providers copies of their VA Northern California Medical Staff appointments in VetPro.
  • Scan privilege lists, then rename scanned documents for each provider by their name, their privilege expiration date, and post their privileges to a shared drive where all VANCHS employees with a need to know have access per joint commission.
  • Post providers in the MS Access Database knowledgeable of pertinent credentialing and privileging laws such as HIPAA,the privacy Act of 1984, VA policies, regulations, By-Laws, and Joint Commission Guidelines.
  • Receive Reappointment packets and New Applications back for review and finalization.
  • Every month send reappointment files to Credentials Meeting for final approval.
