· Responsible for clinical and operational oversight of an assigned home health territory

· Responsible for recruitment and selection of interdisciplinary team members

· Participated in development of Home Health/Hospice policies and procedures

· Successfully lead Home Health program through a CHAP survey resulting in a 3-year accreditation and an ACHC survey also resulting in a 3-year accreditation

· Provide direction to an interdisciplinary clinical team responsible for managing care patients
