- Works alongside the Brooks Rehabilitation Workers Compensation Case Manager in order to assist injured workers and their families discharge into a safe living situation
- Educates patients and their families on a variety of outpatient services provided by Brooks Rehabilitation, including two Skilled Nursing Facilities, a Spinal Cord Injury Day Treatment Program, Brain Injury Day Treatment Day Treatment, Assisted Living Facilities, and 50+ outpatient clinics across the state of Florida
- Daily communication with outside Workers Compensation Nurse Case Managers and Adjusters regarding patient status and any anticipated needs for discharge (medical equipment, post-discharge medical and therapy appointments, transport requirements, etc.)
- Participates in weekly Team Conference Meetings across 4-6 different physicians. Responsible for reporting back to outside Workers Compensation Case Managements on the weekly updates as outlined in each individual conference
The ICDR, or Internal Career Development Residency, is a three month long program offered quarterly that fosters the development of outstanding individuals in the organization, who are committed to careers in health care. Residents posses a strong interest in leadership opportunities, as well as engaging and empowering learning experiences such as:
- Tailored coaching and development opportunities alongside numerous executives within the Brooks Rehabilitation system, as well observing operational tasks as performed by representatives internal and external to the Brooks system.
- High level exposure to system initiatives through executive member shadowing and mentorship
-Provides direct training and exposure to project management, which includes creating, designing, and the eventual implementation of an assigned project
- One-on-one insight into personal and leadership development
PROJECT PROPOSAL: More care is moving out of facilities and into patient homes. The trend is that the very ill will mainly be ones seen in 24/7 facilities. How can Brooks Rehabilitation expand our service offerings more towards the home? Can we treat a certain percentage of our patients currently being seen in the SNF setting in the home instead, with similar if not better outcomes?
● Daily maintenance of a multi-provider schedule, including appointment creation and cancellation for new and existing patients, prioritizing patient care based off of severity of diagnosis, and making as-needed adjustments to provider availability. Also held responsible for regular upkeep of clinic referral log; documenting all incoming referrals from outside providers, indicting when patients were called to schedule – as well as how many times contact was made – and sending update letters to MDs re: scheduling
● Held responsible for overseeing patient insurance benefits and authorizations, including Worker’s Comp, Auto, Medicare and multiple private insurances. Making sure to properly communicate all patient benefit and authorization information at first appointment, as well as collecting payments as directed by patient’s insurance.
● Working alongside multiple off site teams amongst Brooks Rehabilitation, including the billing department, central intake, and provider relations. Working as a singular, multi-faceted unit in order to provide optimal patient and provider relations and customer service.
● Discussing a variety of personal concerns with patients and their families while maintaining HIPAA and PHI standards. Handled all patient issues to the best of ability in person as well as via telecommunication.
● Trained in multiple health information technologies, including TheraOffice, Perceptive Content, Zoom, and Meditech.
