Managed caseload of 20-25 patients weekly with routine follow ups. Performed hospice admissions. Educated patients/caregivers regarding hospice philosophy, disease progression and other aspects of the POC necessary. Reviewed and updated the POC at least every 60 day per reglatory standards. Performed supervisory duties for hospice aides and LPN's and adjusted assignments as needed. Provided physical and emotional care to support the patient and family in periods of crisis.
