Clinical Support Nurse-Home Health | St Luke's Hospital

Job Responsibilities

-Serve as central liaison between field nurse, home health staff, PT/OT, physician offices, pharmacist/anticoagulation clinics, infusion companies and patients to support safe, coordinated care.

-Manage patient-care tasks through EMR and phone/computer-based systems to ensure timely communication and workflow efficiency across the clinical team.

-Enter provider orders accurately into the EMR and verify that documentation meets regulatory and clinical standards.

-Review lab results and promptly relay critical information to physicians, pharmacists, and field clinicians to support timely patient interventions.

-Provide high-level multitasking support for field nurses, therapists, patients, an d physician offices by responding quickly to needs, resolving issues, and removing barriers to care.

-Coordinate care transitions and contribute to smooth workflow operations by acting as a consistent point of communication for all disciplines.

_Support patient and family understanding of care by facilitating information flow, clarifying orders when needed, and assisting with problem solving to ensure safe home health outcomes.

• I worked seven months as a hospice nurse. This was a very special part of my nursing career and I loved being able to be there for the patient and their families. I was able to work during my time off in the summer. After returning to school working hours my availability was only on weekends. I worked almost every weekend. It was a hard decision but I decided to leave Heartland to have more free time.

• Floated throughout five SSM adult hospitals including Pediatric Urgent Care @ DePaul hospital, and later including Cardinal Glennon Children's hospital. Depending on the need of each day determined which hospital (adult or pediatric) area I was sent to. Cardinal Glennon was not part of the SSM registry when I was hired. Having a pediatric nursing background I was sent to Cardinal Glennon Children's Hospital to represent SSM Registry nursing on a trial basis. The plan was to evaluate the RN, "me" on a trial basis for three months to determine if CGCH would agree to use Registry nurse's in the future. When I completed the three-month trial Cardinal Glennon agreed and signed on to use the SSM Registry nurses. I was very pleased to be an integral part of this change/contract.

• Provided direct patient care to children aged infant to teenagers who were admitted for complicated nursing diagnoses, including trauma needing nursing care and rehab before transitioning to home.
• Administered medications via oral, IV and intramuscular injections and monitored responses.
• Tube feedings, trach care and trach changes done as needed. Wound care dressings.
• Loving care, and emotional support given throughout the childs stay.
