  • In management of both Transitional Care Management programs; Hospital reAdmission Reduction Program (HARP) and Complex Care Management (TOC CCM), I consistently interact directly with the CM teams, as well as the local ED, Inpatient, Primary Care and specialty teams for improving care and reducing readmissions for our patients who are discharging from the hospital to a skilled nursing facility or to their home.
