• I am currently responsible for clinic compliance in Medicare Annual Wellness, Hypertension and Diabetes management. I provide leadership to support staff. My daily responsibilities include covering clinic shortages, assisting with prepping patients for exams and procedures. I also assist with procedures, collecting patient data, administering medications. and providing patient education. It is my responsibility to assist with maintaining our clinics compliance with state, federal, local regulations.

Direct patient care. Daily shift assessments. Wound care as ordered. Trained in caring for patients with acute respiratory issues including patient’s on ventilators, trach mist, CPAP, and Bipap. Other duties as deemed by DON.

• Responded to patient requests with appropriate clinical and personal assistance.
• Reviewed patient history to verify conditions and current medications.
• Took samples such as blood and urine to complete diagnostic and routine assessment tests.
• Assessed patients' temperature, pulse and blood pressure and recorded in electronic medical records.
• Documented patient information obtained from interviews concerning symptoms, medications and prior treatment.
• Documented patient vitals, behaviors and conditions and communicated concerns to supervising CRN.
• Delivered oral medications, intramuscular injections and IV fluids to mitigate or resolve patient symptoms. Including administering PPD’s and vaccines.
• Circulated assigned floor to measure and record patients' fluid and food levels, implementing measures to promote healthy intake and output as necessary.
• Clearly and thoroughly explained diagnoses, treatment options and procedures to patients and loved ones, checking in to verify understanding.
• Supported diagnoses by taking blood, urine or sputum samples from patients for laboratory testing.
• Protected patients and healthcare team members from infection by sterilizing equipment and reusable supplies.
• Helped patients complete day-to-day tasks, including bathing, dressing and eating.

Perform nursing procedures according to the standards of care and within the LVN scope of practice as dictated by the California state licensing board, such as:

• Provide unbiased excellent patient care regardless of circumstance. Maintain a high level of patient advocacy.
• Data collection at time of intake to establish plan of care under the direct supervision of a Registered Nurse, PA/FNP, or MD.
• Assessment of drug or alcohol withdraw to initiate treatment as indicated by facility standards and training.
• Laboratory testing: such as blood glucose testing, blood lab draws, dipstick urine, guaiac/hemoculture studies.
• Aseptic and sterile dressing changes as ordered by providers or nursing intervention.
• Prepare and administer prescribed medications (PO, Sub Q, IM, SL, PR, and topical) as ordered by providers.
• Maintain and manage accuracy of all patient records to include MAR's, observations, interventions and plans.
• Recognize and respond appropriately to emergency situations and effectively intervening with emergency equipment, supplies, and materials. Including trauma bag/ AED's and 02 tanks.
• Complete nursing assessment of mental health patients including those in crisis or at risk for suicide.
• Complete and read EKG's as needed.
• Transcribe doctor's orders as necessary and ensure orders are processed and comprehended with pharmacy and non-licensed staff.
• Train new staff including mid-level providers and nurses on daily operations, including policies and procedures.
• Travel to other correctional facilities and train staff on the use of Electronic Medical Records.
• Maintain a professional working relationship with correctional staff and other law enforcement agencies.
• Obtain and maintain security clearance throughout the course of employment.
