Strengthen, rehabilitate and preserve families and provide protection and care for children and/or vulnerable adults in need of supplementary or substitute care, or provide full-time intake services

Evaluate social needs of adults and children, develop a plan directed toward the solution of individual and family problems, and provide protective and placement services for homeless, dependent, or neglected children

• Work includes developing and utilizing all agency and community resources which will carry out the above objectives
• Work is performed under the supervision of an administrative superior, and close supervision is received in the form of review of case work plans, recorded narratives, and frequent case work conferences
• Remain on-call on a 24-hour basis and may be expected to intervene and/or assist law enforcement in volatile situations involving removing at-risk children/vulnerable adults from abusive parents or guardians
• Maintains case history records and keeps records of assigned cases
• Participates in conferences and/or staff meetings
• Supervises foster children/homes
• Confers with clients to develop plans for post treatment activities
• Coordinates counseling efforts with mental health professionals or other health professionals, such as doctors, nurses, and/or social workers
• Makes referrals to other community/agency resources for clients as necessary

• Recruits, studies, and recommends foster homes for approval
• Establishes and oversees administrative procedures to meet objectives set by boards of directors or senior management.

• Conduct individual home visits to individuals or families who need help obtaining community-based services monthly or as needed
• Duties will include obtaining pertinent data of consumers to determine services needed and to make adequate referral and to ensure services are provided
• Duties also include building relationships with school faculty and parents of individuals in program, develop behavioral plans to aid teachers and parents to improve learning and decrease defiant behaviors, correspond with client to create goals and methods of action, help find community resources to aid in needed areas, work with school board to help develop IEPs, create parent seminars to increase likelihood of success
• Communication of hope, and promotion of emotional, behavioral and psychological growth through persistent efforts to attain individual goals
• Promote skills and knowledge to help individuals effectively manage their mental health concerns and develop sense of hope and sense of self that is not illness dominated
• Acted as liaison to community agencies, network with community providers, and seek out new services available to individual or families
• Document accurately and in timely manner consumer progress through progress notes, treatment plans and follow-up contacts according to agency policy and procedure
• Restores, rehabilitates, and supports development of daily living skills, help improve each client’s self-management of negative effects of psychiatric or emotional symptoms
• Assists clients to identify and effectively respond to or avoid precursors that lead to functional impairments
• Provide information and referrals to individuals or families to public or private agencies or community services for assistance
• Monitor ongoing provision of services from community agencies and keep case management plan current with progress of individual or families.

To conduct various processes and specialty groups within the patient program

• Play integral part of assuring that the patient program schedule is carried out as planned by the staff team
• Performs various duties assisting staff nurses in the psychiatric and physical treatment and care of patients in accordance with established methods and procedures of the hospital
• Interacts closely with patients remaining engaged in the various activities and events that patients experience throughout the treatment day
• Always interacts with patients in a therapeutic manner and assists other staff to also maintain appropriate boundaries
• Participates in patient care conferences, substance abuse intervention, observes, monitors, records, and reports patient's progress
• Conducts or assists in conducting group and individual counseling, therapeutic community, and other related milieu activities
• Performs general and special clinical procedures
• Assists patients with nutritional, hygiene, and comfort measures
• Obtains and records vital signs
• Evaluates mental health service activities, collects and updates administrative and statistical data
• Conducts unit self-assessments
• Compiles and prepares medical and administrative reports.

Along with a regular caseload of up to 70 clients: assist in call-in service, persons experiencing mental health emergencies and/or crises, clients or non-clients

• In the event inpatient or hospitalization is needed, meet persons (and/or their family/guardian/etc....) at designated hospital or care facility
• On all nights, holidays, weekends or any time HBHS is otherwise closed for business
• Specialize in areas such as substance abuse, geriatrics or children’s services
• Provide direct service planning and treatment provision as well as client advocacy, community linkage and resource coordination
• Communication of hope, and promotion of emotional, behavioral and psychological growth through persistent efforts to attain individual goals
• Promote skills and knowledge to help individuals effectively manage their mental health concerns and develop a sense of hope and sense of self that is not illness dominated
• Document accurately and in a timely manner consumer progress through progress notes, treatment plans and follow-up contacts according to agency policy and procedure
• Recommend support groups for clients or family members to help them cope with their behavioral health issues
• Maintains financial accounts by processing customer adjustments and recommends potential products or services to management by collecting customer information and analyzing customer needs
• Prepares product or service reports by collecting and analyzing customer information and contributes to team effort by accomplishing related results as needed.
