Hold consultations with future participants and their support group to determine if Aveanna is the correct FMS that will meet their needs. Assist participants through the transition process to ensure a successful transition into the self-determination program. Review participant's finalized budget to ensure that their funds are distributed appropriately to avoid depletion of funds prior to the end of their budget. Reviewed physical and digital records to identify and correct inaccuracies. Stay in constant communication with participant, family, or independent facilitator via email and telephone. Resolved customer complaints, answered policy and procedure-related questions. Reply to all emails promptly to ensure that good customer service is being provided. Review and approve budget renewals and revisions in the order received, making sure that the authorizations are on file so that there are no delays in services.
Assisted patients with activities of daily living such as bathing, grooming, and dressing. Provided basic patient care by taking vital signs, measuring height and weight, and recording intake and output. Reported changes in patient condition to registered nurse or physician. Transported patients to treatment units using a wheelchair. Answered call lights promptly and responded to patient needs accordingly. Turned and repositioned bedridden patients on a regular schedule to prevent bedsores. Observed patients for any changes in their physical or mental condition. Served meals to patients in their rooms and assisted with feeding when needed.
Provide information to families and community resources who have been referred. Assign appropriate referrals to assessment coordinators, who will complete the eligibility process. Receives medical records and reports sent by school to initiate process of intake to determine their eligibility with Lanterman or early start services.
Email or mail informational packets to designated recipients prior to assessment. Receive referrals from parents, including but not limited to physicians, hospitals, social workers, and schools. Complete and submit Purchase of Services. Complete Notice of Propose Action (NOPA) for individual's ineligible for Lanterman services. Complete special assignments upon request.
