Key Job Duties and Responsibilities
Individual Support & Skill Development
• Assist participants in building independent living skills, including communication, socialization, personal care routines, transportation, and safety awareness.
• Transported individuals with care, including those with disabilities or special needs.
• Coach participants in community settings to help them engage confidently and appropriately in social, recreational, and volunteer environments.
• Support clients in setting personalized goals and monitor progress using person-centered planning practices.

Case Management
• Maintain an active caseload remotely, ensuring each participant receives individualized support aligned with their habilitation or program plan.
• Conduct intake assessments, service planning meetings, and regular follow-ups with clients, families, and support teams.
• Document services accurately, including daily notes, progress reports, incident reports, and goal updates in compliance with AbilityFirst and regulatory requirements.
• Coordinate care with outside agencies such as regional centers, healthcare providers, therapists, schools, and community organizations.
• Advocate for participant rights, accommodations, and access to resources that enhance independence and quality of life.
• Assist in developing community-based activities and learning opportunities that promote inclusion, skill building, and social participation.
• Monitor participant safety and well-being during all program activities and respond appropriately to behavioral or medical concerns.
• Serve as the primary point of contact for families, service coordinators, and support networks regarding participant progress and needs.
