  • I was responsible for meals, cleaning, managing appointments, charting, taking vital signs, giving medication, leading through physical therapy exercises, and running errands for this patient.
  • She also needed to be re-oriented to time and space on a regular basis when possible.
  • I reduced severe agitation and anxiety by using care and calming techniques, and providing comfort measures to my patient.
  • Compassionate, focused care allowed me to build trust with this patient and meet her needs quickly.
  • The goal was to give this client as much independence as possible, while still working within her limitations.
  • I stopped working with this client after she was moved to a memory care facility.
