I currently work in the home with an elderly gentleman with severe Parkinson's, CHF, and moderate dementia. I am with him overnight, and my job is to attend to his needs and keep him safe in the home throughout the evening. I monitor his vitals, give medications, and assist this client with sitting, standing, and ambulating. A gate belt and walker are used during ambulation to prevent falls, and I promote independence buy encouraging him to attempt these activities on his own, but provide help immediately when needed. During episodes of confusion or agitation, I re-orient to time and place, or if unable I calm and relax this client until they are peaceful and able to fall back asleep. This client is also assisted with bathing, dressing, toileting, and range of motion exercises. Some wound care is provided due to a pressure sore. I chart and document all meals, snacks, medications, urine and fecal output, possible present swelling or edema, and vitals throughout the night to share with his family and medical team.

I worked in the home with an elderly gentleman who was staying with his daughter and family for a few months after a bad fall and neck injury. I assisted with all activates of daily living including bathing, dressing, walking, and doing range of motion physical therapy exercises. I monitored his vitals, blood sugar, and assisted him with taking his medication. I helped this client with feeding by monitoring his nutritional intake to help him hit weight gain milestones. I charted his activity throughout the day, and gave report to physical therapists, occupational therapists, and his home health nurse regularly. I worked with this client to build a positive rapport, and encouraged him to be as independent as possible while still providing compassionate and attentive care. I also preformed light housekeeping tasks to maintain a clean and pleasant environment in his home.

I cared for an elderly man in his home who needed companionship and help with activities of daily living. I tailored my care to his personal preferences, as this client is cognitively proficient, and mainly requires help with physical needs and tasks. While caring for him my responsibilities included bathing, dressing, feeding, recording vitals, charting notes throughout the day, and using a gait belt to prevent any potential falls or injury. I supervised his medication, provided his meals, and did light housecleaning. I also led him through stretches and physical therapy exercises prescribed by his doctor in order to help with his pain and range of motion.

I worked with an elderly woman in LTC with advanced Alzheimer's who needed help with her physical, emotional, cognitive, and social needs. This client needed help with all activities of daily living including bathing, personal hygiene, and dressing. She required frequent cueing to get through daily tasks due to cognitive defect, which also proved a challenge when making friends. I facilitated this, and helped to greatly improve her social understanding and needs. Due to advanced dementia this client could not always be oriented, so I would find ways to make this work and help her with tasks in the best way possible without causing further confusion or distress to the client. I was responsible for meals, cleaning, managing appointments, charting, taking vital signs, giving medication, leading through physical therapy exercises, and running errands for this patient. She also needed to be re-oriented to time and space on a regular basis when possible. I reduced severe agitation and anxiety by using care and calming techniques, and providing comfort measures to my patient. Compassionate, focused care allowed me to build trust with this patient and meet her needs quickly. . The goal was to give this client as much independence as possible, while still working within her limitations. I stopped working with this client after she was moved to a memory care facility.

I made sure that this client followed her diet and treatment plan, and educated her on nutrition and blood sugar during our time together. Although she had limited range of motion and physical ability, I did my best to ensure her autonomy, privacy, and independence.
