- Coordinating hospice care by working with other team members and patients/family to ensure needs are met.
- Evaluating the effectiveness of hospice services to the patient and family on an ongoing basis.
- Consulting with physician concerning changes in patients’ condition requiring order changes and/or changes in the plan of care.
- Participating in IDG and case conferences.
- Maintaining an on-going knowledge of current drug therapy.
- Conducting a physical examination of the patient, including vital signs, physical assessment, mental status, appetite and type of diet, etc.
- Supervising and evaluating the care given by the Hospice Aide and LPN
- Cooperating with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan
- Monitoring and recording symptoms and changes in patients’ conditions and relaying information to the physician with urgency
- Caring for patients with varying acuity
- Administering medication, including titratable drips
- Advocating for patient's evolving needs and catching safety errors/errors in orders
- Monitoring patient progress, including vital signs and change of condition
- Assisting physicians with procedures, such as insertion of central line or arterial line
- Providing emergency response care and managing a patient’s life support system maintenance
- Participating in patient safety committee and magnet program committee
