• Provide direct, professional nursing care to specified population of patients in the acute care setting; Develops a patient centered plan of care based on individualized needs; Maintains confidentiality of the patient medical record; Counsel and educate patients and their care partner on treatment plans; Administer medication following the five rights of medication administration; Provide wound care and dressing changes, as applicable; Read and interpret patient information, making treatment decisions, when appropriate; Consult with physicians to decide on best treatment decisions when appropriate; Performs nursing interventions to achieve optimal patient outcomes; Observe and record patient behavior and response to nursing interventions; Demonstrates quality, safe and effective patient care; Provides delegation to unlicensed assistive staff and licensed Practical Nurse (LPN) staff; Demonstrates standards of excellence which includes practicing nursing with core values of compassion, accountability, respect, teamwork and integrity; Demonstrates understanding and executes quality and performance improvement plans. (only for RN evaluation); Participates in decision making and takes actions based on ethical principles. (only for RN evaluation); Provides timely and accurate documentation of patient care; All other duties as assigned within the scope and range of job responsibilities.

• Demonstrates skills & knowledge necessary to provide care appropriate to patients in various stages of development; Performs assigned nursing duties for the comfort and well-being of patients; Responds to patient crisis situation & initiates interventions within the scope of responsibilities & notifies RN; Performs successful finger sticks on patients, following all standard laboratory safety policies; Performs defined procedures on point of care instrumentation; Responds with promptness, empathy and interest to needs of patients; Communicates to patients, employees, & physicians utilizing courteous verbal & written communication skills; Demonstrates competency using required computer skills including order entry, admission, discharge & transfer procedures, ordering of tests. Maintains charts according to ChristianaCare guidelines, coordinates maintenance of unit supplies; Completes mandatory in-service education; Performs assigned work safely, adhering to established departmental safety rules & practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients & visitors; Uses proper documentation, writes legibly, signs transcribed orders & records appropriate data; Performs other related duties as required.

• Checking patients’ blood pressure, heartbeat and pulse regularly; Providing compassionate care by listening to patients’ concerns and guiding them by answering any questions or concerns; Monitoring patients’ conditions and providing updates to healthcare team; Turning, repositioning and move patients between beds, wheelchairs and seats; Helping patients use the bathroom and dress; Monitoring patients’ food and liquid intake.

• Assist clients with ADL’s, range of motion exercises, and transfers; Light housekeeping and home management tasks; Vital signs; Assisting with ambulation; Provide companionship; Toileting; Accurate documentation; Monitoring client condition and providing updates to healthcare team;
