PRN Physical Therapist Assistant

Physical therapy treatments completed in short and long term care at Brookestone Village and Brookestone Meadows.

• Provide compassionate care for geriatric patients in skilled nursing facilities, focusing on improving strength, balance, and overall quality of life.

• Planned and carried out individually-designed treatment programs to restore, maintain and improve physical functioning, decrease pain and improve comfort, and maximize mobility.
• Monitored patient progress and adapted treatment plans to reflect changes with communication and consult with the supervising physical therapist.
• Improved patient mobility using upper and lower extremity strengthening.
• Educated families and caregivers on home exercise programs for continuation and carry over of care.

Therapy services provided full time at Brookestone Meadows skilled nursing care facility in Elkhorn, NE treating short-term and long-term care patients, and outpatients. Transitioned to PRN in December 2021- December 2023.

Role for my job is to have continuous communication with my supervising physical therapist regarding my daily patient case load, carrying out the plan of care, progression towards the patient physical therapy goals, home evaluations, modalities, and education to the patient, family, and nursing team for carry over to the patient for successful outcomes.

Physical Therapy services at Brookestone Meadows in Elkhorn, NE.

Carrying out the plan of care provided by the supervising physical therapist to assist the patient to return to their prior level of function, creating and implementing home exercise programs, and education with nursing/patient/caregivers in regards to their individual medical needs regarding functional mobility, training, and safety.

PRN PTA starting in November 2006, and transitioned to a full time PTA from June 2007-August 2008.

Provided patient care in the inpatient hospital setting to also include employment on the acute rehabilitation center with home evaluations, community outings and assessment, education/training with family and patients to assist in achievement in maximizing their function to transition back to home or recommended discharge setting.

Carrying out the patient individualized physical therapy goals outlined by the supervising physical therapist.
