Amanda Edmondson

• Case Management of hospice patients in homes, skilled nursing facilities, assisted living and memory care including delegation, education, communication and coordination with facilities and other hospice disciplines including CNA's, volunteer, Social worker, LPN, Chaplain.
• Advised patients and caregivers of proper wound management, safe medication use and administration routes/strategies, and disease progression.
• Promoted patient and family comfort during hospice admission and end of life processes.

• Explained course of care and medication side effects to patients and caregivers in easy-to-understand terms.
• Offered exceptional care and support to individuals recovering from acute incidents and dealing with chronic conditions.
• Assessment and documentation, and medication administration for 4-5 general medical and step down patients with frequent admissions, discharges, and changing patient assignments.

• Care and documentation for 3-5 cardiac and/or general medical-surgical patients with the management and stabilization of rapidly changing patient conditions in cardiac step-down unit.
• Frequent post procedure assessments as well as recurrent monitoring and of patients on continuous cardiac medication infusions.
• Initiation and titration of cardiac medication infusions.
• Quality completion of patient care and tasks in a very busy, constantly changing environment with numerous interruptions and distractions.

• Collection of nasopharyngeal and throat specimens with operation of Abbott ID Now machine.
• Occasional triage via Covid Hotline and vaccine administration.
• Triage and patient intake at Urgent care Clinic.
• Ability to produce successful outcomes while working both autonomously or with a small team functioning together in close quarters.

Night Shift Charge RN and Staff RN

• Management of staffing plan and patient assignments for day shift for both nurses and nursing assistants.
• Successful communication with other departments and House Supervisor related to staffing plan, coordination of admissions, problem-solving and conflict mitigation with patient behavior escalation.
• Leadership and delegation Registered Nurses and Certified Nursing Assistants with plan of care, resources, patient care, patient and staff advocacy, and conflict resolution and mitigation.
• Compassionate care , assessment, and documentation for 4-6 acute Medical-Surgical patients from pediatric to geriatric often with multiple co-morbidities recovering from illness, injury, or surgery (gynecologic, urologic, orthopedic, plastic, cardiac, and general surgeries).
• Strong patient advocacy and thorough patient education.
