My position as the Nurse Advance was to be the Nurse for Sierra County and currently covering Socorro County. In this position, I see clients for Annual Exams which include providing clinical support for Pap Smears, Breast Exams, Breast and Cervical Cancer Screening, and birth control. I participated in the Needle Exchange, Overdose Education and dispensing of Narcan to clients.
As the Nurse Advance for Sierra County and covering for Socorro County my duties include Lab Coordinator, Pharmacy Coordinator, and VFC Coordinator. I was responsible for the daily activities and assignments within the office.
The Lab Coordinator duties include performing Quality Controls on lab tests, ordering stock, maintaining staff competencies, licensure's, and trainings. I
As the VFC coordinator I maintain logs of vaccines given, daily temperature logs both paper copy and the New Data Loggers. I order, return and complete reconciliation reports of vaccines.
I helped set up and coordinate an Open House, where the community was invited to learn more about services provided as well as receive informational packets. I have also participated in different outreach opportunities such as National Night Out and a Veterans Health Fair where flu shots were given.

As a member care coordinator, I have assisted members with Medicaid. I work with members and help assist members with Personal Care Services, Environmental Home Modifications, and assisted with finding resources and providers. In this position, I used basic nursing skills including a head to toe assessment, rapid assessment. I triaged patients as they enter the clinic and assign each patient a level of acuity based upon nursing assessments. I screened patients, and complete a more in-depth assessment based upon vitals and symptoms. If the client had a higher acuity level I could complete a warm transfer to the ER/Urgent Care. If the client was to be admitted or flown out to a large facility I could complete the warm transfer. I can work with the providers to provide and complete care based upon patient needs. I can calculate medication dosages based on standing orders with the pediatric population. I can place IV's, saline locks, venipuncture's, accu-checks and wound cultures. I also can give immunizations to the pediatric patients following CDC and VFC guide lines. Our department had received an award from the CDC for immunizing 90% of our patients. I would float to the Urgent Care Clinic and assist with triage and clinical support for the clinicians. I also floated and provided assistance to Family Medicine Clinic for the intake of patients.
In this position, I have been assigned to PI Projects, including hand washing, recruitment, mock code drills, and Joint Commission. The Hand washing Project started in our clinic and data was collected and PDSA's. We were trying to maintain a 90% satisfaction with hand washing. We will then approach this with the hospital and try our process in other clinics. The recruitment project was to see if we could obtain more applications outside the normal job postings. With this project, the clinic has hired a Nurse, through the advertisement on Facebook and social media. There were in-service trainings that I provided for the clinic, Alaris IV Pump usage/troubleshooting and latest information regarding HPV vaccine.
I have also been assigned as Charge Nurse, where I make daily assignments and handle any issues that may arise in the clinic. I can manage staff and patient needs of the clinic. During my time of being a charge nurse, the hospital's Electronic Documentation went down twice, I was fully aware of the hospital's policy to revert to paper charting. Working with the providers and staff, the clinic continued providing excellent patient care with little to no interruptions. I would also train all new staff and contract nurses. I maintained their competencies, orientation to the clinic and ensured they completed reviewing the clinic's policies and procedures. I used the bulletin boards in the waiting area to provide community teachings as well.
I had my NM Nursing state license, BLS, and ACLS certification. I also have completed Baby Friendly Training for General Healthcare Personal. I am familiar with EHR and documentation. I was a RN on the medical surgical floor. I performed direct patient care. I can obtain vital signs and know what values are in normal range and complete a head to toe assessment, perform pre-op and post-op assessments, teachings and patient care. I can administer medications to patients based upon the five rights of medications, and recognize high alert medications, including IV piggy back medications, IV push medications, SubQ, IM, intradermal injections, PO, rectal and topical medications, as well as blood products. I can perform veni-punctures for labs and obtain an IV site, as well as maintaining central lines. I can receive and give report on patients. I can provide discharge teachings, on the illness, medications, and equipment. I can perform a wide range of dressing changes from ulcers, skin tears, surgery, trauma, burns, wound vac, ostomy appliances, and perform staple removal. I can place a patient in traction and splints. I can also insert, maintain, and administer medications with a NG tube and suction patients as needed. I can straight cath, place a Foley, and perform irrigation of the bladder. I can alert the MD for signs and symptoms that are out of the normal and assist the MD with bedside procedures. I can navigate through EHR to chart (admissions, transfers, discharges, post-op, observations, progress notes, shift notes), perform chart audits, place telephone orders and verify doctor orders.
I was also placed as charge nurse and assigned duties to staff. My Supervisor would have new staff or contract nurses follow me and I would orient them to the floor and train them. I created a "HOW TO" guide for staff. The guide included how to troubleshoot and use the EHR and the COWs (computers on wheels) as well as different programs needed to complete our job on the floor. Such as what needs to be charted, how to electronically dispense medications, use the IV pumps and wound vacs.
I have floated between the two Medical Surgical floors and floated down to Urgent Care clinic where I was assigned to triage patients. I can complete MSA work with putting a patient into EHR, build and breakdown charts.
