  • Responsible for maintaining a client caseload of approximately 120 clients. Required to independently manage and provide services such as individual medication management/individual therapy, administration of long acting antipsychotic injectables, and Clozapine clinic which requires monthly labwork monitoring, assessment and coordination of services of over 30 clients.
  • Daily collaboration with multidisciplinary treatment team to revise goals, objectives and interventions based on patient status and needs. Multidisplinary team includes support staff, licensed social workers, and four on site psychiatric providers. Responsible for maintaining, updating, reviewing, and documenting each client's treatment regimen under the Office of Mental Health and the Department of New York State's guidelines that is assigned to caseload. Treatment plans are reviewed on a regular basis with individualized goals and criteria for discharge implemented into care. Responsible to update appropriate providers on changes to prioritized assessed needs, results of required regular screening tools (SBIRT, Tobacco screen, DAST, PHQ-9, Lethality assessment, AIMs, Cognivue results), and to update these changes in treatment plans.
  • Administer long acting antipsychotic injectables such as Invega Sustenna, Abilify Maintena, Risperdal Consta, Haldol Decanoate, Prolixin Decanoate and Aristada to a high risk population group requiring monthly medication management and follow up to ensure therapeutic effect of each prescribed injectable.
  • Responsible to update legal and community services such as Assisted Outpatient Treatment Program, the office of Social Security and Disability Serices, and community case management providers on changes to services or any non-compliance with treatment regimen.
  • Daily communication and referrals to a variety of community resources in order to meet each individual client's needs. All areas of health are addressed including social, financial, emotional, physical and mental health. Ongoing communication with community resources such as home health agencies, primary care clinics, substance abuse clinicis, shelters, food programs, outpatient day treatment programs, case management providers, transportation servies, SPOA, insurance providers, supportive housing programs, and acute rehabiliation facilities.
  • Responsible to intervene with high risk clients requiring a higher level of care; responsible to update Crisis Services, Adult Protective Services and any other program involved with assisting clients in a community setting.
  • Responsible to include family members or other members of client's psychosocial support system to participate in treatment regimen with overall goal of maintaining client's stability and independence at all times.
  • Ongoing physical health monitoring. Responsible to review and sign yearly physicals for all clients linked with clinic. Required to provide health education recommendations based on health screenings and ensure recommendations are addressed with client by linked clinician.
  • Required to obtain vital signs and assessments each in person visit and on an as needed basis for providers ordering acute assessments of overall mental and physical status.
  • Responsible to review and keep track of annual labwork required by insurance providers such as CMS (Centers for Medicaid Servics) for all clinic clients.
  • Responsible to communicate with inpatient hospital staff for appropriate discharge from inpatient hospitalization and linkae to outpatient clinic to ensure continuity of care and adhrence to treatment/medication regimen.
  • Respnsible for ensuring all clients obtain prescribed medications and obtain them from appropriate pharmacies, through proficient knowledge of prior authorizations, billing codes, and reequirements of documentation for reimbursement purposes.
  • Daily use of telehealth services with appropriate documentation via Office of Mental Health/Department of Mental Health guidelines and within compliance of HIPPA. Responsible to schedule telehealth health monitoring visits with appropriate billing codes/documentation requirments.
