As a member of the leadership team, my daily duties included admission review, submitting or obtaining PASRR prior to admission. Completing LOC, Submitting Medicaid applications and consistent communication with billing department. Reviewing admission rules/ responsibilities with new residents. Began discharging planning with resident at admission. Discussed plans moving forward- long term or to return home. Managed all discharges home or to other location. Assisted DON in helping with daily schedule for CNA's, monitoring that charting was completed for the week and help CNA's responsible if something needed completed. Assisted CNA's on floor if needed help from management. Addressed all resident/ family complaints or concerns. Communicated effectively with Admin and DON of any updates or changes. Did many marketing events in the community with corporate Marketer to establish our name in the community and for job recruitment.

Started as field Nurse completing home health and hospice visits in home throughout 8 counties. Monitored and documented on but not limited to: wound care, Medication management, foley/ cath care, end of life/ Hospice care and education for patient and/ or family as needed. August 2023 Promoted. Began Marketing for company. Increased online presence through google and social media. Planned many events in the community to educate on not only our facility but the importance and benefits of Hospice. Assisted company in designing and picking proper locations for future billboards. Did many events within facility as well to increase staff participation and show appreciation. Along with Marketing maintained all PR issues that arose in facilities or hospitals in the area. Once notified of concern, visited location to discuss and plan resolution. Communicated any updates/ changes with Administration. Assisted Volunteer coordinator in helping build and manage Volunteer Program.

Coordinated nursing care on unit through staff assignments, assisting and rounding with physicians, monitoring patient orders, and communicating with ancillary departments for up t o20 residents. Completed medication pass, wound care, hygiene needs and education for resident/ family as needed. Worked alongside PT/OT and Management on collaboration of care to achieve best outcome as possible for resident. Worked in Restorative Nursing as well with therapy department. Maintained daily schedule to meet all resident needs to continue with therapy goals once therapy was complete. Notified therapy of any changes/ decline.
