• Obtain FSBS and administer insulin

as ordered-report very abnormal findings

to provider so that he or she is aware for

possible new standing orders

• Sign in medications when pharmacy

delivers-accurately sign out narcotics as

they are ordered

• Successfully change G-tubes and f/c’s every

30 days or as ordered

• Performed phone triage on incoming patient calls and determined severity of injuries and/or sickness
• Scheduled appt with PCP or urgent care for non-emergent related callls. per nursing guidelines and protocols for triage care.
• If this nursr felt like they needed ER, this nurse would contact EMS for transport to local ER
• Coordinated preparation of medication refills for PCP
• Delivered comprehensive education to patients and families on care adjustments/care plan initiated by PCP

• Interviewed new patients and entered medical information into EHR for provider
• Routinely updated appt adherence tx plan
• Met with newly diagnosed patients every 2 weeks to review and ensure their understanding of their care
• .Provided prevention education to newly STI diagnosed pts and reported the infection to state epidemiology dept

• Tested at risk walk in patients for HIV and HCV and provided education as well as placed referrals if needed
• Met with HIV positive patients and screened for eligibility for enrollment in HIV state funded programs such as KHCCP and KADAP
• Assisted in applying for SSI and/or SSDI
• Assisted in applying for medication assistance via pharmaceutical programs
• Collaborated with and sent referrals to other specialty clinics
• Provided resources such as gas cards and med vouchers to pay for their medication

• Delivered comprehensive care for one individual in both home and school setting
• Administered prescribed medications utilizing G-tube
• Conducted tracheotomy maintenance and care
• If change in status, immediately notified her PCP and family

• Delegated duties to CNAs optimizing team efficiency
• Administered prescribed medications and treatments per provider orders
• Observed and if change of care, promptly notified PCP as well as family
• Tailored treatment plans based on clients' health changes
