
I am a dedicated very enthusiastic LPN with 18 years of nursing experience offering
proven skills in direct and in acute settings as well as long term care settings. I enjoy being a nurse and helping others. At the end of my shift, it feels rewarding knowing that I changed at least one life during that shift. I have been told that my work is
meticulous and that I am a team player as evidenced by my willingness to always help others. I personally feel that I am
devoted and go above and beyond to ensure that patients needs are met. I want every patient and family member that I
encounter to be a positive and memorable experience , regardless if I was their nurse that day or if the encounter was

residents and monitor CNA staff to high
standards of patient care and safety
at this long term care facility Report changes/decline in status to medical
provider, DON, and family
• Assess skin; if wound discovered, measure
the area.and contact provider,, DON, and
family immediately.
• If new treatments and meds.
If they don’t foresee any problems or
concerns, then this nurse will enter the order
and fax to pharmacy
• Obtain FSBS and administer insulin
as ordered-report very abnormal findings
to provider so that he or she is aware for
possible new standing orders
• Sign in medications when pharmacy
delivers-accurately sign out narcotics as
they are ordered
• Successfully change G-tubes and f/c’s every
30 days or as ordered
