As Population Health Nurse, I find myself often working independently. I have taken charge to learn as much as possible regarding the ACO and CMS requirements for care coordination and medicare annual wellness visits. I have worked on our current workflows to make sure that we are capturing required information that is needed for billing. I have worked to understand Cerner and how to use it work to our advantage in the ambulatory setting. In this Population Health Nurse position I am able to use my communication skills disperse information to the other nurses.

As an Emergency Room nurse, I often find myself in a leadership role. I work under very stressful situations and can stay calm and collected to help keep the newer nurses on task. I have shown my leadership skills by being selected as a fill in Nursing Supervisor. While fulfilling this role I can direct nursing and hospital staff in the tasks that need to be fulfilled.

As Population Health Nurse, I am able to work with the other nurses in the Ambulatory setting. I currently hold and lead meetings to give updates in workflows or changes in our current EHR. I am able to work as a team and bring an open mind when listening to opinions and ideas of other team members.
