I am responsible for providing admission, case management, and follow-up skilled nursing visits for home health patients. I work closely with the physician to develop an initial treatment plan based on their orders and the patient's initial assessment. I provide hands-on care, management, and evaluation of the care plan and teach the patient under physician orders. I coordinate with various healthcare personnel, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers, to ensure appropriate care. Additionally, I report the patient's care, condition, and progress continuously to the patient's physician. I work with the patient and their family to implement the care plan and assist them in achieving optimal resolution of any needs or problems.

As an RN case manager, I implemented and reviewed healthcare plans. I advocate for patients and their families and focus on providing efficient and effective health care while managing treatment costs. I am currently a certified preceptor and have obtained the Wound Care-Associated Certificate.

I provided, coordinated, and directed home health nursing services and case management processes according to the assigned patient’s individualized plan of care based on agency policies and procedures through the competent application of the nursing process.

I recorded and undated patient medical information, vital signs and maintained detailed and accurate reports. Consulted and coordinated with other health care team members to plan, implement, and evaluate patient care plans: educated patients and their family members on the disease process and medical treatments.
