Responsible for providing admission, case management, and follow-up skilled nursing visits for home health patients. I work closely with the physician to develop an initial treatment plan based on their orders and the patient's initial assessment. To ensure appropriate care, I coordinate with various healthcare personnel, including physical therapists, occupational therapists, speech therapists, medical social workers, home health aides, and external providers. Additionally, I report the patient's care, condition, and progress to the patient's physician. I work with the patient and their family to implement the care plan and assist them in achieving optimal resolution of any needs or problems.

As RN case manager, I implemented and reviewed healthcare plans. I advocated for patients and their families and focused on providing efficient and effective health care while managing
treatment costs. I was certified as a preceptor and obtained a wound care certification. I trained many advanced registered nurses in this field.

I provided, coordinated, and directed home health nursing services and case management processes according to the assigned patient's individualized plan of care based on agency
policies and procedures through the competent application of the nursing process.

I provided and coordinated patient care, educated patients and the public about various health conditions, and provided advice and emotional support to patients and their families. I administered oral, IV and IM medications and monitored patient reactions.
