Intake Coordinator

  • In my current role, I receive Referrals from all Case Managers throughout Colorado, as a Home care Agency we cover all 64 counties and client's in the community. I as well work with the VA, Private Pay Clients, and Region 10 program. I work closely with Case managers for Options for Long Term care, reviewing referrals, speaking with potential new clients, and providing details and information about client needs to the correct agency. Peoplecare care has a North, South and West Agency divided amongst Colorado.
  • As Intake Coordinator, I receive daily Inquiry calls from community members throughout the state in need of services. I work with the caller to determine if they have Medicaid, if they are a VA or if they qualify for Private Pay services. I provide options to support the member’s needs, provide steps on how to apply for Medicaid LTC and if possible, work with their case managers for their referral to be sent to our Agency.
  • I follow all accepted referrals from start of Intake to Start of care services, communication with all agency team members and case managers. Ending with a 30-day new client quality call to ensure their services meet their needs, they have a positive communication with the agency, and they are happy with the care they are receiving.
  • In addition to my role as Intake Coordinator, I attend Monthly State Stockholder meeting, as well take detailed notes to report back to our CEO and leadership team. Meetings consist of updates to Medicaid and State Regulations, Programs under Medicaid updates, changes coming by the State and the Department of Health Policy and Finance
  • I provide support with interviewing Open Care Coordinator positions for all 3 Agencies and working closely with the Agency Managers. With the shortage of Care Coordinators in our North Agency, I have been filling in as a Care Coordinator since July to ensure our Clients in the Northern Region of Colorado are receiving quality care.
  • In 2014, I was hired on as a Care Coordinator carrying a Case load between 55 to 100 clients. My responsibilities where to oversee each client's care, building supportive care plans for caregivers to support a community members home care needs. Working with caregivers, providing training, working with case managers, and providing in home visits with clients every 90-days
  • I was then promoted to Assist Agency manager then shortly after to Agency manager. As Agency manager I over saw a team of 13 plus that consisted of schedulers, care Coordinators and timesheet analysis. I worked the weekly Medicaid billing Report, worked new Referrals, ran weekly team meeting, joined my care coordinators out in the field for "ride Alongs", provided monthly census reports to leadership, interviewed, hired, and terminated staff. I worked closed with State regulations and the nursing team for our skilled clients, along with close relationships with case managers for the West Slope of Colorado.
  • I was then moved to Financial Analyst where I worked closely with our Billing team, worked weekly Over Under reports, data entered all Members State approval PARs into our Client system, reviewing Timesheets and ensured clients were receiving their full state approved hours.
  • Prior to moving into the Intake Coordinator Role in January of 2023, I was an IHSS Program Assistant. My role was working closely with our RNs, care coordinators and our IHSS (Skilled) Clients. I reviewed assessments, care plans, visit summaries, client Documents to ensure that all pieces of our IHSS Clients met state Regulations. This was a new position when I moved into the IHSS Role and was provided the approval to create and formalize the IHSS Program job role, responsibilities, and process.
  • During my almost 9 years with Peoplecare, I have organized our yearly Annual Trainings with the support of leadership, organized and been on a team where we supported Community events such as Alzheimer's Walk, Delta County Fair booths , community booth events, Food Bank, Blood Bank, Team events such as parties for holidays and birthdays, meet with County Single Entry Point Case management teams, and collaborated with the State in problem solving difficult client cases. As well have I been a part of several State Surveys , Medicaid Billing Audits, Support with our computer soft wear program enhancements, Agency process development and training staff .

  • As Staff Development Coordinator, I worked closely with the RN, LPN, and CNA team members. I provided all their schedules, was a part of hiring, along with termination of CNA's. Bi-Weekly I provided All Staff Team meeting which covered about 200+staff members and Monthly provided State of Texas required trainings.   • Our Local High School provided a CNA program, and I would oversee the group of students that would participate in the CNA Shadow program, working with their preceptors, the students and setting up their day-to-day schedule.

During my years living in Aspen, I catered small events and gatherings for Aspen community members, catering to each client's requests adding small details to enhance their event, and adding my own style to ensure their occasion was a memorable one. I believe in the smallest detail makes the largest impression. As well, while living in Aspen, I volunteered at the Aspen Camp School for the Deaf coordinating outings and participating in camp events with the children. I had the pleasure for the summer months managing the housekeeping team at Mt. Chalet, meeting guests’ requests and needs to set up dinners, outings, and activities. During my time Rocky Mountain Institute, located in old Snowmass, I cared for the landscape of the institute, worked with the horses, and greeted guests who came to tour the Institute. Prior to relocating to Hotchkiss, I worked for Cattle Movers located out of Carbondale where I was the chef and events coordinator to put on daily lunches for the Cattle Drives, Lobster Bake for the local Rodeo and set up Teepee Village for our overnight quests. My passion continued when I moved to Hotchkiss and I catered private small dinners as well as worked with Leroux Creek Vineyards for weddings, small to larger events, parties and guests providing an artwork of foods, to table decorations and enhancing the natural ambiance with lights, candles, and flowers. As a member of Delta County Fair board for many years, I organized and held the yearly Delta County Jr Rodeo event, along with worked with the events team for the nightly events as well as events in the park.  On many occasions for family friends, I have organized and set up for baby showers, weddings, family gatherings , graduations and parties.
