  • Assess client eligibility for program services, complete intakes, and create individualized service plans.
  • Provide services identified on the service plan in a timely manner.
  • Make necessary referrals within the agency and outside of the agency, as needed.
  • Maintain client files, update case notes, and update calendar.
  • Ensure that all client information and activities are tracked in Client Track as well as Refugee Management Solution.
  • Collaborates with Resettlement Agency to support the overall team goals.
  • Works collaboratively with local health departments, social services offices, mental health providers, school systems, SNAP, USCIS, ESL and other service providers to establish referrals when appropriate.
  • Perform other duties as assigned, such as assisting with outreach, training volunteers and interns, assisting with projects.
