Provide compassionate, high-quality daily care and assistance to residents in a skilled nursing and rehabilitation facility, ensuring safety, dignity, and comfort in accordance with individualized care plans.
Supported needs of 10 residents with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, ambulation, feeding, and oral care, promoting independence where possible.
Take and record vital signs (temperature, pulse, respiration, blood pressure, oxygen saturation) and report significant changes or abnormalities to licensed nursing staff promptly.
Accurately document care provided, resident status, and intake/output in electronic health records (EHR) according to facility protocols and HIPAA guidelines.
Provide assistance during rehabilitation therapies by helping residents move to/from therapy sessions and supporting their participation in prescribed exercises.
Monitor and report physical, emotional, or behavioral changes, signs of pain, or deterioration to the charge nurse to support timely interventions and resident safety.
Maintain infection control standards by adhering to hand hygiene, proper use of PPE, and environmental cleaning protocols in compliance with CDC and facility guidelines.
Promote a positive and respectful environment by engaging residents in therapeutic conversation, recreational activities, and memory care support.
Assist in safe resident transfers using gait belts, Hoyer lifts, and other approved equipment following facility transfer protocols and fall prevention guidelines.
Collaborate with interdisciplinary care teams (nurses, therapists, dietary, social workers) to ensure holistic and continuous care.
Respond promptly to call lights, resident requests, and emergencies, demonstrating excellent time management and prioritization in a fast-paced healthcare setting.
Educate and support families with understanding care routines, facility processes, and resident updates under the guidance of nursing staff.
Maintain clean, organized resident rooms and workspaces; replenish linens, hygiene supplies, and assist in transporting residents to meals or appointments.
Attend regular in-service trainings and staff meetings to remain compliant with state regulations, enhance clinical skills, and stay updated on best practices in long-term and rehabilitative care.
