• Initiates and facilitates appropriate drug dosing and manages medication-related pharmacist Consults.
• Participate in clinical activities and documentation such as IV TO PO, renal dosing and pharmaokinetics.
• Educates Providers and nurses on possible drug interactions, potential side effects and optimal methods of administration.
• Completes critical medication order, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems.
• Participates in completing Admission medication reconciliation(AMR) for all inpatients admins and Discharge medication reconciliation (DMR) for all Medicaid patients.
• Actively provides coverage of the technician services provided by Central Pharmacy and IV Room
• Mentors new staff and assists with training of all staff as requested. Supports experiential training of pharmacy trainees and mentors pharmacy students (IPPE and APPE) as requested/required.
• Supervises technician and intern practice, prioritizes work load, organizes work flow, and checks accuracy of pharmacy technician activities.

• Performs patient medication therapy monitoring and disease state management counseling
• Initiates prescriber communication regarding suggested changes for medication therapies
• Partners with the patient to solve for non-adherence issues as well as work with the patient and the prescriber to assess the need for supplemental medications based on the unique needs of the patient and their disease state
• Proactively conducts analysis of care setting drug utilization data to identify trends, develop strategies, and implement comprehensive programs to improve patient outcomes
• Advises patients about general health topics such as diet, exercise and managing stress or refers to appropriate professional and Promotes medication safety and compliance

• Counsels patients and answers their questions regarding usage of medicine, side effects, interactions, contraindications, patient information privacy, generics, less expensive medicines, over-the-counter products, and refers to medical provider as needed to ensure medication is taken correctly, health needs addressed, and satisfaction with service.
• Performs pharmacist tasks including compounding, drug therapy reviews, verification, and medication management. Reviews, interprets and accurately dispenses prescribed medications when necessitated by workload.
• Follows-up with medical providers’ offices to clarify prescribed medications, dosages, refills, interactions and allergies to suggest alternative medications, and answer medical provider questions
• Performs retail, clinical, and wellness services such as immunizations, flu shots, diabetes awareness and other preventive healthcare services.

• Making phone calls to patients to complete comprehensive medication reviews (CMR), fast pace environment, coping with difficult situations, documenting different patient interactions, and counseling patients on different conditions and medications.
