Summary

For almost 11 years I have worked in the managed healthcare field. I was a Care Coach for 7 years and my role involved triaging patients to help prevent a hospital and ER visits. I followed chronic care patients in hopes of catching a decline early and working with the referral coordinator to get the HHC/DMEs that patients would need.

I then got promoted to Transitional Care Nurse. My role is to see members or their families in the facilities and discuss a safe discharge plan such as HHC, custodial care of hospice. I work closely with the Humana UM and the facility case management to ensure members have HHC/DMEs arranged before discharge.
