I am responsible for many different task. I process FMLA forms, Global Referrals, RPO referrals. I process orders for DME, wound supplies and incontinence supplies. I do direct patient care through our Share The Care Program and the RN Blood Pressure Titration Program. I refill medications for patients, do monthly medication reviews which requires me to check all medication expiration dates and discard medications that are old. Responsible for ordering all medications provided for our patients such as: Flu, Pneumonia, RSV, Covid, Shingles, Childhood immunizations, etc. I monitor the In-Basket in Epic daily for patient request and communications to the nurse and for the doctor and process them. I also triage calls from patients and appropriately determine a resolution.
I am considered a SOC nurse for this company. I open the patient and then pass the patient on to another nurse. I provide instruction to the patient/family regarding appropriate care after discharge from the hospital or rehab facility. I instruct on medications, appropriate diet related to the diagnosis, any wound care required, IV instructions/infusions, PICC line or mediport care. Pain pump instruction and care of the dressing after a TKA. Responsible for contacting the doctor to reconcile medications, diagnosis, disciplines ordered, frequencies and any other orders that might be needed.
I was a Nurse Case Manager and my responsibilities were to :
Familiarize patient/family with the home care process and services to be provided and the relevance to the patient. Responsible for collaboration with all of the disciplines on the case to make sure the homecare experience goes smoothly. Educate and provide instruction to the patient/family on diet, medications, disease process, importance of exercise, use of infusion pumps, glucose meters, INR machines, oxygen tanks, peg tubes, wound care. I am proficient in the use of PICC lines for blood draws and infusions, instructing the family on how to flush the PICC line and maintain the patency of the line. I am proficient in staple/suture removal, complex wound dressing care. Provide support for the patient/family and prepare them for discharge from home care.
