Connected homeless population with shelter access, housing a variety of resources around the portland area.

Assisted homeless population with accessing Transition Projects Day Center services. Including mailroom services, showers, laundry services, clothing, lockers and Oregon ID assistance.

• Accompany participants experiencing homelessness to appointments in the community, medical, DMV, DHS, and Social Security Office via public transportation, medical transportation or cab.
• Participate in regular team meetings to improve participant care.
• Provides trauma-informed care for participants, to manage stress and anxiety on public outings.
• Consult with medical staff and TPI wellness team to eliminate barriers to care and identify areas of concern for participant management intervention.
• Navigate successful transpotation from shelter to hospital settings back to shelter, making sure all specific needs were met.

• Supervise shelter activities to ensure safety of participants.
• Provide advocacy, information, resources and referral services to participants.
• Responsible for participant intakes, maintaining accurate logs, shift reports, and participant records, both manually and using web-based program.
• Skilled at problem solving and providing crisis intervention services.
• Displayed sensitivity to cultural needs of participants.
• Encourage participants through relationship building, one on one emotional support, and goal setting.

• Supported women and children transitioning out of homelessness, drug and alcohol addiction, trauma, mental heath challenges and domestic violence partnerships into stable housing.
• Oversee reception/staff desk and supervision of all program participants, volunteers and open DHS case child visitation.
• Modeled a clean and sober healthy lifestyle, while building relationships with program participants
• Encouraged participants in their goals and growth with coaching, support and enforcement of the program guidelines and safety procedures.
• Supervised program activities, mobilized volunteers and lead participants so all participants remain focused on their individual tasks, growth plan and relationship building goals.
