Roles And Responsibilities

  • Identifies and determine the priority of patient's problems and needs through continuous observation and thorough Nursing Assessment (Body Systems Approach) to both acute and chronic, critically ill patients who have Neurological, Cardiovascular, respiratory, Gastrointestinal, Renal, urological musculoskeletal and vascular problems as it is a adult critical care area.
  • Renders comprehensive Nursing Care to the Critically Ill patients utilizing the Nursing Process (Assessment, Planning, Intervention, and Evaluation) accordance with established priorities toward preventive, therapeutic, and rehabilitative aspect of therapy.
  • Reviews and carries out Physician's Medical Orders correctly and efficiently and keeps them up to date with the patients' health status.
  • Administers Medications utilizing the Standards and observes the 10 Golden Rules in Drug Administration.
  • Evaluates patients' reactions to drugs being administered, drug to drug and drug to food incompatibilities.
  • Assists in the provision and maintenance of adequate and functioning equipment such as Infusion Pumps, Suction Machines, and Oxygen Delivering devices.
  • Facilitates and assists in Diagnostic and Therapeutic Procedures, noting results and patients' reactions.
  • Relays pertinent laboratory results and refers conditions needing immediate or in emergent response.
  • Monitors and documents patients' vital signs correctly, noting for abnormalities in bedside and central monitoring units. 12 lead ECG to be taken and uploaded in the system whenever changes in the rhythm.
  • Prepares the patient for transfers to step-down units and for discharge, accompany with the patient for CT and MRI.
  • Educates the patient and the family regarding the disease, treatments and medications taken stressing the importance of follow-ups and compliance to the treatment regimen.
  • Documents patients' progress utilizing the SBAR Format in the Nurse's Progress Notes.

  • Receive the hand over from outgoing team member, prioritize the patient acuity and visit them accordingly. Improve the recognition of deterioration, provide intervention regardless of location to improve patient outcome.
  • Attend the patient as per MEWS Score (Modified Early Warning Score)
  • Reduce the number of cord blues outside the ICU and unplanned admissions
  • Guide, teach and support clinical staffs in managing complex situations.
  • Accessing porta-Cath, perma-Cath and central lines in wards and emergency department.
  • IV cannulations in difficult situations.
  • Arranging and escorting critical patients to I.C.U
  • Administration of high alert medications and narcotics
  • Post ICU patients' follow up in the ward for 72 hours, review lab values, vitals and medications if any escalation or electrolytes replacement requires, will contact the concerned physician for orders.
  • Liaise with nursing supervisor, plans and agree on priorities at huddle times.
  • Meet the team leaders of ICU and emergency department -note high risk patients and reassess MEWS before transfer to ward.
  • Commence ward rounds to vulnerable group.
  • Accept call from wards needing advice and attend as required- document/discuss actions.
  • Respond to adult code and other system requirements as directed by nursing supervisor or excising call out system.
  • Collect data on activities and actions and provide summarized written report to group email at the end of the shift.

  • Provide clinical advice, assessment, action plans and where appropriate interventions to ensure quality patient care.
  • Maintain all competencies and skills as required to maximize utility and support across a board range of clinical environments.
  • Guide and support clinical staff (including non-nursing) staff managing complex situations.
  • Provide bed side teaching advice and guidance where possible.
  • First responder for code blue. Respond to other emergencies as requested.
