- Establish comprehensive care plans and work with interdisciplinary team of nurses, doctors, therapists and dietitians to ensure highest level of care.

- Complete intake assessments, social history, elopement risk, aggressive/harmful behaviors, suicidal/homicidal ideation, abuse/neglect screening, discharge potential and inform of resident rights.

- Assist in interpreting social, psychological and emotional needs of the resident/family to the medical staff, attending

physician and resident care teem members.

-Discharge planning including coordination of home health services, durable medical equipment, caregiver services and meal assistance.

-Manage staff in social services department and oversee operations.

-Site Supervisor and mentor for graduate level interns studying psychology.

Member of safety and quality assurance committees.

-Staff development on topics including resident rights, abuse and neglect, cognitive loss and elopement.

-Work with interdisciplinary team comprised of nurses, doctors and therapists.
