Primary Conduit for all physician/patient communication and documentation of same via electronic medical record, managed all patient medication refills, administered injections, primary provider of patient education relative to all clinical test results, disease management, adjustments to medications and treatment recommendations, and submissions of Prior Authorization forms to insurance carriers for denied medications.

Provided in home care for two separate geriatric patients in their homes as needed.

Supervision of Social Work Staff and RN Case Managers, implementation of treatment plans and revisions, completion of psychosocial assessments, participation in Treatment Team with psychiatrists, coordination of discharge planning activity, provision of individual and group therapy and documentation of all interventions in the medical records. Continue to provide social work consultation to the ICU, Acute Care floors and the Emergency Department. Member of the Ethics Committee, Utilization Review/Quality Improvement Committee and the Critical Care/Emergency Department Committee.

Provision of individual therapy to assigned clients one day per week in Dr. Urrea's out-patient clinical setting. Completion of treatment plans and outcome of individual therapy session reports. Administration of intra-muscular psychiatric medications.

Following the implementation of Case Management and the integration of the Utilization Management and Social Services Departments, assumed all r

les and responsibilities assigned to the Utilization Management staff, including medical chart review, procurement and authorization for acute care, skilled care and continued stay from managed care entities, daily consultation with all assigned attending physicians relative to the plan of care, as well as all discharge planning duties. Also provided periodical consultation and coverage to the hospital based Skilled Nursing Unit, as well as the Home Health Department, Intensive Care Unit, Acute Care and Emergency Room.

Supervision of social work staff, implementation and oversight of departmental policies and procedures relative to discharge planning, annual budget development, continuous performance improvement duties, participation in annual State and JCAHO surveys and direct practice. Provided health education to local school districts and civic groups, employee assistance services to Hospital employees and periodic group work. Also performed utilization management and discharge planning duties.

Provision of social work services and discharge planning for acute care population as well as emergency and outpatient departments. Status as a Licensed Practical Nurse mandated daily performance of utilization management duties and sole provision of these activities during absences of the Utilization Management staff.

Duties included provision of various physical therapy modalities under the direction of a licensed physical therapist in addition to clerical duties.
