As a Nurse Assessor I perform telehealth and in-home evaluations for individuals seeking long-term support and services while documenting the assessment in the NY-UAS. I review all their medical documentation, health referral forms, and medications. I initiate communication between the Call Center as needed. I enhance communication and collaborative relationships with interdisciplinary care team members to improve care coordination and facilitate service delivery. I attend monthly team meetings. I have new Nurse Assessors and Quality Assurance Nurses Shadow me for training purposes, and I have precepted other Nurse Assessors.

I worked as a relief daytime Supervisor and also as a relief Head Nurse when necessary. As Supervisor, I was responsible for the coordination of the care of 115 residents and staff. Responsibilities include following up with acute residents and communicating and receiving orders from the MD. Coordination of care is between residents, therapy, social work, admission coordinator, MDS Coordinator, activities, and administration. When scheduling was off, I was also responsible for making sure the building was adequately staffed.
As the Charge Nurse I was responsible for the care of the sisters that had skilled needs and Dementia, along with the assisted living sisters. The responsibilities included admitting new sisters to the facility, and transferring those that were ill to the hospital, and desired end of life care. I was responsible for assessing and evaluating the needs of the sisters, keeping in contact with the MD, receiving orders and providing them as ordered, passing medications, and providing wound care. I kept in contact with the MD, nurses, pharmacy, PT, OT, dietary, and activities daily to make sure the sisters needs were being met.
As the nightshift supervisor, I was responsible for the care and safety of 100 residents, LPNS, and CNAs. I reviewed charts, double checked monthly MARS, checked in medication orders, provided wound care, and made sure the next shift was staffed and had their assignments.
I worked as the Education and Training Coordinator developing and providing classes to teach adults to be PCA/HHA. As part of the classes, I was responsible to monitor the supervised clinical component to ensure timely completion of the training. I assisted with the training-to-work transition and follow up for new staff. I also was responsible for developing and providing mandatory in-service for all agency staff.
I worked as a Community Health Nurse, managing a caseload of 18-25 clients. I made home visits to assess and evaluate patients needs for homecare services. These services include admitting the person to homecare, providing care, educating about their diagnosis, teaching medication and symptom management. As a CHN, I had to stay in contact with the MD, pharmacy, family, PT, OT, aide and the patient to assist the patient in following their plan of care.
I worked as the Supervisor of the Olean office and was responsible for the aide and nurse supervisions and evaluations, on site education of staff, disciplinary action, and interviewing prospective employees. I also managed a caseload of homecare patients and hospice patients, and was responsible for admitting and discharging them from services. I was responsible for teaching patients about their diagnosis, medications, symptoms management, and pain control.
I was responsible for performing allergy skin tests and administering allergy shots. I was also responsible for the daily activities of a busy medical office such as setting appointments, writing scripts for the MD to sign, taking vital signs and patient histories.

As Head Nurse, I was responsible for admitting and discharging residents, review MD orders, assessing and evaluating residents with any change in medical status. I was also responsible for managing two LPNs and eight CNAs. As Supervisor. I was responsible for managing six LPNs, and 24 CNAs, 3 Head Nurses and 120 residents. Responsibilities as Supervisor included, but was not limited to maintaining the safety of the staff and the residents and making sure the Pines was fully staffed.
