Primary Care Duties:

• Perform physical examinations, health screenings, and comprehensive assessments.
• Diagnose and manage acute and chronic illnesses.
• Develop and implement individualized care plans.
• Prescribe medications and manage treatment regimens.
• Promote preventive care including immunizations, lifestyle counseling, and screenings.
• Order and interpret diagnostic tests (labs, imaging, etc.).
• Educate patients and families on conditions, treatments, and health maintenance.
• Refer to specialists when necessary and coordinate overall patient care.

Wound Care Duties:

• Conduct wound assessments and develop evidence-based treatment plans.
• Provide advanced wound therapies such as debridement, wound vac therapy, compression therapy, and dressing changes.
• Monitor healing progress and adjust treatments accordingly.
• Collaborate with interdisciplinary team members (e.g., RNs, surgeons, dietitians) to optimize wound healing.
• Educate patients and caregivers on wound prevention, management, and infection control.
• Document wound care and healing progression accurately in the EHR.
• Participate in quality improvement initiatives related to wound outcomes.
• Conducted comprehensive patient assessments to develop personalized care plans.
• Collaborated with multidisciplinary teams to optimize patient outcomes and streamline care processes.

• Conduct initial and ongoing assessments of hospice patients, including physical exams, symptom evaluation, and psychosocial needs.

• Manage and prescribe medications for symptom control and palliative care under applicable state laws and collaborative practice agreements.
• Provide education to patients and families regarding disease progression, symptom management, and end-of-life expectations.

• Responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation, and evaluation;
• Provides direct patient care within the scope of practice; Directs and guides patient/family teaching and activities of other nursing personnel while maintaining standards of care in Direct Observation Unit;
• Communicate with the physician about changes in the patient’s clinical condition, including cardiac monitoring and diagnostic studies results.
