As MDS Coordinator , I assess and monitor proper treatments for residents in a nursing home. I meet with the IDT team to come up with individualized care plans based on the assessments set by CMS. I review patients charts , MDs progress notes , nurses documentations , MARS and TARS to capture pertinent information for reimbursement purposes. I also report findings and make recommendations to management with regards to patients care. I also attend Daily Utilization Review meetings , Weekly and Monthly Triple Checks and Case-Mix Meetings. I update 672 and 802 Forms.

• Monitored patients' conditions and reported changes in physical presentation, appearance, and behavior to discuss treatment with physicians.
