Obtain data for nursing assessment from patient/family. Identify patient care problems and establish priorities to provide patient care based on patient care plan. Assist in coordinating activities needed to implement patient care plan. Evaluate patient response to care provided. Respond in a timely manner to changes in patient condition. Escalate issues when demands exceed capacity. Provide effective teaching to patient and family. Document in a clear and concise manner.

Patient population consists of respiratory failure with tracheostomy and ventilators. Maintain patent airway by providing tracheostomy suctioning/care. Collaborate with respiratory therapist to meet the needs of patients. Administer medications, perform wound treatment/prevention. Maintain patency of g-tubes and administer feedings/medications via g-tube. Evaluate and respond effectively to changes of condition of patient and report to MD.
