• Provide nursing and clinical care for residents, correctly and according to their care plans
• Assist residents with the safe administration of medications
• Recognise and respond to change in the condition of residents' health & well-being
• Complete simple wound dressings
• Implement continence programs and update TENA wardrobe cards and prescription
• Attend to routine checks, i.e. blood pressure, temperature, pulse, blood sugar, FNO,RAT testing, following GP directives
• Review and assess a resident post fall, completing all paper work as per policy.
• Communicate with allied health care profesionals including PT.OT,SP Dietician, GP/NP, DSA, MPaCCS.
• Assist in managing residents with diabetes including updating assessments and following GP/NP directives
• Maintain accurate documentation throughout each shift
• Supervise staff in the provision of effective person centred care to our residents with the expected outcome of positively enhancing their quality of life
• Respond to the requests and concerns of families and carers in liaison with the Residential Manager and Clinical Nurse Manager
• Provide nursing and clinical care to residents through development, implementation, review and evaluation of care plans
• Ensure AN-ACC assessment and referral records are maintained

I also completed 12 months of Clinical Nurse support 2 days a week where my tasks included the above and

• Case conferences
• Care Plans
• Stock ordering
• Continence care under TCMS ( prescribing and ordering)
• Working with families and GP's to reduce the use of Psychotropic medications
• Toolbox education with staff
• Quarterly quality indicators
• Monthly weights
• Following up reports from Dietician, SP and GP's

• Provide assistance to residents with their hygiene needs including; bathing, dental hygiene, dressing, grooming and personal care.
• Support the maintenance or restoration of residents' independence by providing assistance to engage in activities of interest and use mobility aids where required.
• Complete accurate and timely documentation and report information to relevant Nursing, Allied Health and Management as required.
• Maintain effective communication in the workplace, being respectful, open to change and kind in your approach.
• Assist residents with the administration of correct medication in accordance with Juniper policies
