Care Manager
- Assist clients in developing and achieving short-term and long-term goals.
- Provide advocacy for clients to ensure their needs are met through appropriate services.
- Coordinate with other health care providers, such as physicians and psychiatrists, to develop comprehensive treatment plans.
- Conduct assessments of client's physical, psychological, cognitive, social, economic and environmental needs.
- Develop service plans based on assessment findings; monitored effectiveness of plans regularly.
- Maintain accurate records of client visits and progress notes in accordance with agency policies and procedures.
- Organize recreational activities that promote physical activity and positive interactions among peers.
- Ensure that all documentation is completed accurately according to state regulations.
- Maintain daily living standards by assisting clients with personal hygiene needs.
- Attend and actively participated in meetings to provide and receive information on patient progression.
- Monitor care plan to evaluate effectiveness, document interventional achievement and suggest changes.
- Monitor vital signs and documenting variances, and concerning responses.
- Develop and maintain positive relationships with clients, families, and healthcare providers to support integrated care.
- Conducte initial assessments and periodic reassessments of clients' needs and eligibility for services.
- Maintaine comprehensive records of client assessments, care plans, and progress notes in compliance with regulatory standards.
