  • Evaluated patients upon admission to Lyn Rehabilitation using evidenced based practice. Patients undergo a through examination of body structure and function using objective measures to identify impairments and functional limitations.   • Caregiver training and education provided to identified person(s) throughout inpatient rehab stay. Caregivers are instructed in how to assist patients with functional mobility activities while maintaining proper body mechanics. caregivers are also trained on safe and appropriate use of recommended durable medical equipment prior to discharge home.
  • Provided patient/caregiver with supplemental education as appropriate and in consideration of learning style, language preference, cognitive and physical abilities. Patients are instructed in individualized home exercise programs to ensure carryover of gains made during inpatient rehab stay.
  • Demonstrated safe patient handling techniques in accordance with patient specific needs. Appropriate use of specialized equipment to ensure safety of patient, caregivers and support staff.
  • Responsible for training new staff members onboarded to LRC7 and expected to provide direct patient care (nurses, personal care technicians). Regardless of prior experience, staff members given hands on training related to importance of maintaining good body mechanics, proper patient handling, transfer techniques and safe use of equipment. Additional training and education provided on positioning and handling techniques for patients with specific weight bearing precautions, spine precautions, total joint replacement, stroke, spinal cord injury, sternal precautions, amputee, etc. Staff members completed competence check list and provided additional training as needed.

Initiated education sessions for nursing and support staff focused on understanding typical precautions and contraindications when working with specific diagnoses or post surgical issues.

Initial evaluations, daily treatment notes, weekly progress notes, interdisciplinary team conference notes, discharge documentation and recommended durable medical equipment.
