  • My work performance duties were to perform and participate in the admission of the youth. I also assist in the completion of the admission summary, and I gathered the information that was provided by service providers.
  • I was also part of the discharge planning for the youth.
  • I would put a discharge plan in place with the input of the patient, the parent or legal guardian, and the treatment team member.
  • The discharge plan would consist of the placement, their educational needs being met, all their medication in place, their case management appointments set up, and a therapeutic treatment plan in place.
  • I would also develop their individual aftercare plan, addressing clinical follow-up service needs of the clients in conjunction with the discharge plan.
  • When it came to their discharge, I would prepare and process referral packets, meet with their collateral service providers, arrange for patients' transfer to wherever they are going to be discharged, coordinate services prior to discharge, and verify at the time of the discharge.
  • I would service their liaison to other departments, providers, and referral sources relative to the discharge planning and patient discharge.
  • Provide documentation of the patient and activities completed as part of the discharge process.
  • I also did all the weekly documentation for medical records. This included the following: parental/legal custodian, family contacts, collateral contracts, telephone contacts, site visits as needed, services provided, discharge planning, utilization of community resources, linkage of other programs that the patient can use, and progress/barriers in treatment, as well as progress/barriers towards discharge.
  • I also provided treatment planning input in the development of treatment plans regarding the basis for establishing short- and long-term goals.
  • Prepare pertinent documents, and make recommendations regarding treatment planning.
  • Attend and participate in weekly multidisciplinary treatment team meetings.
  • I would make recommendations, discuss the discharge plan, and provide information for discharge and the after-care plan.
