Triages veteran when they present to clinic as unscheduled visits, document thoroughly in the computerized record their primary concern. Utilizes nursing protocols and Julie Briggs as a guide to determine the best course of care. Report to the provider for appropriate disposition as needed. Initiates contact with all veterans assigned who have been diagnosed with ambulatory care sensitive conditions to provide ongoing patient education. Follows up with veteran recently discharged from hospital to confirm they are fully informed of discharge instructions. Administer vaccines, Depo-Vera , injections, asthma treatment via nebulizer and fill in as chg nurse when needed and act as patient advocate.
