  • Achieved success with discharges up to and beyond 80% compared to last year we were at 30% with getting Spine surgical patients discharged before 11:00am; With the Safe home recovery program we have improved significantly and have been successful in getting 80% of Surgical spine patients discharged by 11:00 am. This is a group of patients that up until last year were staying an average of 2-3 days.
  • Developed improvement tracking and assessment system to outline fall outs from the Safe home recovery .
  • Spearheaded initiative to improve documentation by the nursing team on the 4th floor. I Took the lead and collaborated with team which included ANM Lydia, Surgeons from Ortho surgical team and Neurosurgical team as well as the PACU nursing team, Radiation department, Physical Therapy department, IT team, Nursing managers and Physician assistants. Also including the front line Nursing staff.
  • Coordinated with the radiology department to provide a transporter at 0700 am so that patients could be transported down to radiology. As to not delay the discharge. Through this program we found that X-ray was causing a huge delay for discharging early.
  • Facilitated getting a DME closet on the 4th floor to prevent delays through having equipment readily available.
  • Devised new program to optimize the 4th floor nursing in understanding the Safe Home recovery program. By providing a information sign to hang in the nursing unit. To help nursing staff who might float to the 4th floor understand the safe home recovery program.
  • Collaborated with the team to develop tracking tool to track and document Spine SHR pre surgical calls.
  • Improved departmental collaboration by working with a multidisciplinary team through the Model Cell program to devise a Team communication rounding best practise.
  • This is now used Hospital wide as the new team communication rounding method.
