  • Traveled to clients' homes to complete healthcare services and promote continuity of care.
  • Provided mobility assistance such as walking and regular exercising.
  • Evaluated clients to identify and address wounds, behavioral concerns and medically relevant symptoms.
  • Assisted clients with dressing, grooming and feeding needs, helping to overcome and adapt to mobility restrictions.
  • Followed nutritional plans to prepare optimal meals.
  • Assisted clients with living independently through skill-building and mentoring in areas such as housekeeping adaptations and preparation of healthy meals.
  • Administered prescribed medications under direction of the clients nurse.
  • Cared for clients with diagnoses such as respiratory failure, diabetes, Parkinson's disease and muscular dystrophy, cerebral palsy and several others. Some clients may not have had any at all but just need your help assisting them with certain things.
  • Checked vitals such as temperature, blood pressure and blood sugar levels.
  • Trained new employees/home health aids whenever I was needed to do so. They would come with me daily to each of my clients' homes and follow along with me step by step until they were finally ready to take on their own clients.
  • Running errands for certain clients that couldn't do them on their own such as; picking up their medications from the pharmacy, getting needed essentials and/or groceries from the store in a timely manner.
  • Using good and polite communication skills when speaking with the clients and the clients' family members. Having patience with them was very important.
  • Always making sure my phone or the clients phone was in reach in case of a possible emergency, if or when my supervisor needed me. Whether it was a possible problem with a client, a question regarding a client or for a possible new client that needed to be seen if i was able to. Maintaining the stability of your schedule was very important.
  • Reported unusual or urgent circumstances in any clients' condition or environment immediately to a nurse.
  • Completed entries daily on my flow sheets that my company provided. I always followed the care plans to accurately document and report each clients' progress. Then turned in all my flow sheets to my supervisor for each client by the end of the work week.

  • I successfully completed the internship and maintained a high level of patient care at my internship there.
  • I shadowed my CNA trainer as she did her day to day patient care.

  • Each day I also, charted each patients activity and vital signs.
