• Provides support to the individual to ensure objectives are met in accordance with their individual treatment plans.
• Direct hands on hygiene support (grooming, bathing, ear, oral, manicure, incontinence care). Changed dressings, bandages, maintained sanitary measures.
• Provided mobility assistance in public/personal spaces. Utilized GPS to locate individual.
• Prepared healthy meals to support nutritional needs, assisted with feeding.
• Engaged in meaningful conversation, socialization and activity while providing personal care assistance.
• Maintains daily progress notes and any other appropriate documentation in accordance with company policy, program standards, or other regulatory policies
• Ability to create, follow and communicate consistent schedules and routines for the client and the household.
• Must provide transportation and accompany individuals receiving services to planned and/or necessary activities and appointments – via personal vehicle, bus, etc.

Advocate for client’s needs for all medical, dental and/or psychological/psychiatric appointments.

This includes supporting the clients to communicate needs and desires to the providers and collecting adequate documentation - including after visit summaries, prescriptions, additional service referrals and required follow-up of the encounter.

• Supported/tracked daily hygiene, chores, meals to increase independence.
• Directed/assisted individual/group exercise for enhanced client health.

• Medication Administration
• Provides training, assistance and/or supervision to individuals in the areas of daily living, safe behavior, personal care, financial management, community and social integration and/or job skills development.

• Assisted with personal care appointments such as haircuts and doctors', Provided Direct Hands on Hygiene Support (hair, skin, ears, nails)
• Administered medications and helped client set reminders to learn how to take important medications according to schedule for (2) two years to ensure health/increase independence. Result = Client no longer needs medication assistance.
• Educated client about important nutritional habits and how to plan healthy meals while staying in budget.
• Coached client through difficult life circumstances promoting self confidence.
