- Advanced Illness Management
- Analyze and interpret patients’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses
- Facilitate collaboration among interdisciplinary team members in the plan of care for patients with acute/chronic illness
- Provide consultation in pain and symptom management and advanced care planning
- Telemedicine assessment to determine clinical status, plan of care, symptom management, effectiveness of treatment plan
- Determining the effectiveness of the treatment plan with the documentation of patient care outcomes
- Reassessing and modifying the plan as necessary to achieve health and medical goals
- Provide palliation and symptom management related to chronic illness
- Consult with attending staff as requested so that symptom management for patients is effectively and expeditiously accomplished
- Prepare and maintain accurate patient records, charts and documents to support the medical practice and reimbursement for services provided
