Conducts comprehensive member psycho-social assessments through root cause analysis and performs clinical intervention through the development of a care management treatment plan specific to each member with high level acuity needs. Manages members with severe mental illness who have high rates of behavioral health utilization and/or severe psycho-social vulnerability. Assess members to recommend activities and behavioral routines to meet the social and medical needs of members and their families. Provides support and/or intervention and assists members in understanding the implications and complexities of their current medical situation and/or overall personal care.

Assess and evaluate patients who are incarcerated at either Falkenburg Road Jail or Orient Road Jail. Complete an initial mental health evaluations and provide recommendations for treatment such as psychiatric follow up for medication or discharge planning services. Assess patients daily on suicide watch in the infirmaries and lock down. Conduct weekly group sessions with patients in the mental health stabilization unit. Provide assistance with sick call request for patients, including crisis intervention and death notification. Evaluate patients for Baker Acts to determine if inmate requires involuntary commitment upon release. Consult with psychiatrists, medical staff and other mental health staff on best course of treatment for patients.

Completed Comprehensive Behavioral Health Assessments for court ordered children in the Hillsborough County Foster Care system. Interview, observe and review information related to children who are court ordered into out of home care in the Dependency Court. Consult with school personnel, counselors, parents, case managers and other professionals to determine recommendations for the child while they remain in out of home care. Provide DSM diagnosis under Medicaid guidelines.

Began my employment as an intake specialist meeting with patients upon intake to Northside Mental Health Center. Completed intake interview with patients and diagnosed according to the DSM . Placed patient in appropriate follow up with psychiatrist and therapist for counseling services. Patients seen with appointments and walk up services. Consulted with psychiatrists and therapist to determine patient's best course or treatment.

As a forensic counselor my responsibilities were to enroll patients who were found incompetent to proceed in Northside Mental Health Center's Forensic In Jail Program. Provide counseling services to patients prior to transport to court ordered commitment facility to restore competency. Provide follow up services such as counseling and medication management upon return from commitment facility. Consult with psychiatrist and forensic specialist on course of care for the patient.

Worked with Fitzgerald Middle School students who had been suspended from school for infractions ranging from skipping class to cussing out staff. Program was designed to assist the youths with not falling behind academically and developing coping skills and to avoid further suspensions. Individual and group counseling was available to the students and telephone communication was held with the student's parent.

Supervised a case load of children court ordered into foster care through the Dependency Court. Worked with parents and care givers to provide the best care for the child while under court supervision. Attended court hearings, staffing and monitored visitation with children and parents. Prepared court documents for review by the Attorney General's Office. Consulted with school personnel, counselors, medical staff and other professionals involved with the case.

Began my career at the Department of Children and Families as a counselor in Economic Services, then was promoted to a Foster Care Counselor Position where I provided services to children in foster care. Eventually promoted to an Adoption Supervisor where I over saw a unit of counselors who were responsible for locating forever homes for children who's parental rights were terminated and were available to adoption. While in the Adoption Supervisor position was responsible for supervising adoption subsidy payments to adoptive parents in Hillsborough County. Also responsible for monthly adoption statistics for three adoption units.
