Perform hospice face to face visits to assist IDT with recertification of terminal condition. Provide insight and timeline of patient’s condition and status to ensure conditions of participation are met. Make suggestions for care and interventions to optimize comfort at the end of life.

Integrative care nurse practitioner at various post acute rehab programs, SNFs, ALFs as well as a small primary care patient panel from community. Perform routine exams, diagnose, and manage acute and chronic conditions in collaboration with the patient’s primary care and specialist physicians. Work closely with leadership at facility to ensure goals and needs are met. Coordinate review of Medicare part A and Rehab to Home patients and follow up after discharge. Community based primary care services of small panel of clients outside of my routine facility based tasks.

I have held many positions at various companies under this enterprise. Assisting where I was needed. Including but not limited to several hospice leadership roles; CEO and administrator of their home health company and my role in primary care as an NP providing primary care with a patient panel at clinic and several ALFs, SNFs and LTCs. I have been responsible for developing and operating the home health agency including obtaining and maintaining Joint Commission Accreditation. All my roles involved developing various programs exclusively and collaboratively.
