• Administered medications to residents in a timely manner while doing 3 administration checks and checking the medication rights to prevent any errors.
• Signed in and counted narcotic and sedative medications with another medication aide and a nurse, to ensure a correct count.
• Took any vitals needed prior to administering medications (blood pressure, pulse etc.), explained each medication to each resident, and ensured they swallowed all prescribed medications.
• Contacted nurse, HCP, and/or pharmacy when residents were low on medications, had new medication orders, and/or changes to existing orders.
• Identified any medication errors and immediately reported them to the nurse.
• Monitored for medication side effects, and for changes of residents' overall condition and reported to the nurse.
• Kept the medication cart cleaned and organized.
• Collaborated with other members of the healthcare team (PT, pharmacy, HCP and others) to ensure adequate and safe patient care all around; and to also discuss any updates and/or changes made to residents' care plan.
• Set dining room up for meals, assisted residents to and from dining room, and cleaned dining room up after meals.
• Trained new employees on medication aide policies, and resident care policies and procedures, as well as safety protocols per Brookdale and the state.
• Contacted residents' families, HCP, and the nurse anytime a resident had an injury whether from a fall or other form of injury.
• Documented on computer and a paper communication log changes in residents' conditions or changes in medications.
• Maintained a clean and safe environment by adhering to Brookdale's protocols as well as the states protocols.

• Managed multiple delinquent accounts for medical debt collectors, and followed up with the customers on any unpaid debt.
