• Provide Diabetes Education outpatient full time. The timing of your job posting was ironic and it is my desire to work Part time. I begin at URHCS full time on March 25th, 2024.

As a direct report to the CMO, Dr Walker, I performed comprehensive assessments, CGM, insulin therapy-pump and nutritional guidance and education. I provided patient education, goal setting and follow up with participates and coordinated this care with participant/provider. I assisted in planning, implementing and evaluating the DSME program, coordinating group activities and support groups. I assisted in monitoring and facilitating activities and policies to maintain AADE recognition and compliance with DSRIP guidelines. I performed data analysis, managed monthly operational expenditures and purchases and proactively sought new opportunities to increase operational efficiency for short and long term company objectives while making timely decisions that are result driven.

• As a direct report to Dr Dunn, endocrinologist, I performed comprehensive Diabetes Self management Education as well as insulin pump and CGM therapy. I performed all aspects of program management in accordance with AADE guidelines. I assisted in preparation of CGM data analysis, speech writing for Dr Dunn, and performed community, provider and nursing Diabetes education according to hospital goals.

• As a Direct report to CEO, Joyce Crumper I developed the Pulmonary rehabilitation, Diabetes Education and WorkSteps program for Bowie Memorial Hospital. My experience over my career at BMH allowed me to serve as the Director of Rehab, Director of Home Health and ultimately the Director of Cardiac, Pulmonary and Diabetes Education. I performed all program management in accordance with program guidelines and Medicare/Medicaid standards. I led the departments and managed overall operations with a keen focus on higher performance, quality patient care and efficiency. I ensured completion of site work within defined timelines in coordination with managers, IT, and team leaders. I performed direct patient care in Cardiac, Pulmonary and diabetes performing comprehensive assessments, providing education, assisting in exercise prescriptions and completing and submitting required documentation according to standards of care for reimbursement. I began the Diabetes Education program in 2008.

• As a direct report to med surgical floor manager and house supervisor I provided assessments and care of pediatric, pre and post natal patients as well as post surgical patients.
